That's a great question. I was on the Ten from 1985-2005 and saw all the scheme changes but never heard where the original came from. I will research. BTW, the last Ten was delivered in 1988 and McD did not merge with Boeing until 1997; none us ever considered it a "Boeing" airplane.

The original paint scheme was a McDD creation. 

The next two schemes were AF driven.

 This is the answer I received from one of the initial cadre pilots. 

“It was a combination of DAC/AF.  CMSGT Greg Oveson was responsible for the flag on the tail and the stripes on the side.”

Whoever designed the delivery scheme for the KC-10 deserves the thanks of a grateful nation.  The best looking operational paint scheme ever.  Not withstanding the Loewy scheme on AF1.
